• Up-to-date immunizations

Out-of-State School District:

• Up-to-date physical within the past 365 days

  • Pre-Preparation Physical Form

• Up-to-date immunizations

Out of Country:

• Up-to-date physical within the past 365 days

  • Pre-Preparation Physical Form

• Up-to-date immunizations

• TB test with results (if applicable to country)

Local Health Departments:

• Sussex County Health Department (Sussex County Residents)-

  • Free vaccines are administered to school-aged children and teens (through 18 years old) who have no insurance
